    <ns1:description language="sr">Uvod: Cilj rada bio je ispitati tačnost kompjuterizovane tomografije (CT) i magnetne rezonance (MR) udetekciji lokalnog i metastatskog recidiva pankreasnog adenokarcinoma (PAC) nakon hirurškog zahvata,u poređenju sa pozitronskom emisionom tomografijom sa kompjuterizovanom tomografijom (PET-CT)kao referentnim standardom i analizirati povezanost između funkcionalnih MR i PET parametara.Metodologija: Uključeno je 97 pacijenata koji su podvrgnuti CT, MR i PET-CT pregledima u svrhupraćenja nakon radikalne operacije karcinoma pankreasa. Nakon otkrivanja ili sumnje na recidivkarcinoma pankreasa CT-om, urađeni su PET-CT i MR pregledi. Očigledni (apparent) koeficijentdifuzije (ADC) lokalnog recidiva je koreliran sa veličinom (maksimalni prečnik) i funkcionalnim PETCTparametrima: srednjom i maksimalnom standardizovanom vrednošću apsorpcije (SUVmean,SUVmax), metaboličkim volumenom tumora (MTV) i ukupnom lezionom glikolizom (TLG).Rezultati: Poređenjem rezultata CT i PET-CT pregleda utvrđeno je osrednje slaganje u detekcijilokalnog recidiva (Kappa 0,45), slabo slaganje u detekciji metastaza (Kappa 0,15), a poređenjem MR iPET-CT pregleda dobro do odlično slaganje u detekciji lokalnog recidiva (Kappa 0,467–1,000). MRdijagnostika je pokazala veću dijagnostičku pouzdanost (73–100%) u detekciji lokalnog recidiva uodnosu na CT (75%). Utvrđena je jaka koleracija između maksimalnog dijametra lokalnog recidiva savrednostima ADC i PET parametara. Takođe, ustanovljena je značajna negativna korelacija izmeđuvrednosti ADC i svih PET parametara, izuzev SUVmax.Zaključak: MR sa sekvencama difuzije (DWI) i dinamskim postkontrastnim sekvencama pokazujevisoku dijagnostičku pouzdanost u otkrivanju lokalnog recidiva karcinoma pankreasa u poređenju saPET-CT. Pouzdanost CT je nešto manja, u okvirima literaturnih podataka. Numerička vrednost ADCmape potvrđuje postojanje lokalnog recidiva. Postoji značajna korelacija DWI, kao funkcionalnogparametra MR sa standardnim i naprednim PET parametrima, kao i svih funkcionalnih MR i PETparametara sa veličinom lokalnog recidiva.</ns1:description>
    <ns1:description language="en">Introduction: The aim of the study was to examine the accuracy of computed tomography (CT) andmagnetic resonance imaging (MRI) in detecting local and metastatic recurrence of pancreaticadenocarcinoma (PAC) after surgery, compared to positron emission tomography with computedtomography (PET-CT) as the reference standard, and to analyze the correlation between functional MRIand PET parameters.Methodology: Ninety-seven patients who underwent CT, MRI, and PET-CT scans for postoperativemonitoring following radical pancreatic cancer surgery were included. After detection or suspicion ofpancreatic cancer recurrence on CT, PET-CT and MRI scans were performed. The apparent diffusioncoefficient (ADC) of local recurrence was correlated with the size (maximum diameter) and functionalPET-CT parameters: mean and maximum standardized uptake values (SUVmean, SUVmax), tumormetabolic volume (MTV), and total lesion glycolysis (TLG).Results: Comparison of CT and PET-CT scan results revealed moderate agreement in detecting localrecurrence (Kappa 0.45), weak agreement in detecting metastases (Kappa 0.15), while comparing MRand PET-CT scans showed good to excellent agreement in detecting local recurrence (Kappa 0.467-1.000). MR diagnostics demonstrated higher diagnostic reliability (73-100%) in detecting localrecurrence compared to CT (75%). There was a strong correlation found between the maximum diameterof local recurrence with ADC values and PET parameters. Additionally, a significant negative correlationwas established between ADC values and all PET parameters, except SUVmax.Conclusion: MR with diffusion-weighted imaging (DWI) sequences and dynamic post-contrastsequences demonstrates high diagnostic reliability in detecting local recurrence of pancreatic cancercompared to PET-CT. The reliability of CT is slightly lower, within the range of literature data. Thenumerical value of the ADC map confirms the presence of local recurrence. There is a significantcorrelation between DWI, as a functional MR parameter, with standard and advanced PET parameters,as well as all functional MR and PET parameters with the size of local recurrence.</ns1:description>
