  <dc:description xml:lang="srp">Cilj: U razmatranju etiologije hallux valgus deformiteta sve češće se usmjerava pažnja na endogene etiološke faktore, koji se odnose na morfološke specifičnosti koštanih i zglobnih struktura prvog niza prednjeg dijela stopala. Ovo istraživanje je posvećeno analizi uticaja odnosa dužina prve i druge MT kosti kao i morfologije prve MT kosti na distalnom nivou te mogući uticaj morfologije prvog MTC zgloba na povećanje HVU i IMU, koji definišu ozbiljnost ovog deformiteta.Materijal i metod: U prospektivnu studiju uključeno je 167 pacijenata kod kojih je utvrđen hallux valgus deformitet i 111 ispitanika bez ovog deformiteta. Nakon kliničke obrade urađeno je standardizovano radiografsko snimanje stopala i potrebna mjerenja primjenom licenciranog digitalnog programa. Tako su izvršena mjerenja dužina prve i druge MT kosti distalno od Maestrove linije, utvrđena radiografska forma glave prve MT kosti i pozicija njene zglobne površine mjerenjem DMAU, orijentacija baze ove kosti i stepen inklinacije njene zglobne površine mjerenjem PMAU. Utvrđena je forma prvog MTC zgloba mjerenjem DCAU i izmjereni HVU, IMU i IFU, te apsolutne dužine prve MT kosti, proksimalne i distalne falange palca. Primjenom odgovarajućih statističkih analiza ispitana je povezanost svake od navedenih nezavisnih varijabli sa vrijednostima HVU i IMU, a potom i njihov uticaj na veličinu ovih uglova.Rezultati: Utvrđena je statistički značajna korelacija između razlike dužina prve i druge MT kosti sa IMU, kao i značajan uticaj na vrijednosti ovog ugla na nivou p=0.01, dok uticaj na HVU nije potvrđen. Zaobljena forma glave prve MT kosti prisutna je kod 82.6% stopala sa deformitetom, dok je kod zdravih podjednaka zastupljenost zaobljene (43,2%) i kvadratne forme (42.3%). Pokazana je statistički značajna veza između forme glave prve MT kosti i HVU (Sig.=0.004), ali i značajan uticaj na vrijednosti ovog ugla (Sig.=0.000), kao i manje značajna veza sa IMU (Sig.=0.012) i uticaj na njegovu veličinu (Sig.=0.047). Za razliku od zdravih, kod stopala sa deformitetom dominiraju umjerena i naglašena lateralna angulacija zglobne površine glave prve MT kosti koja ima statistički značajnu korelaciju sa HVU (Sig.=0.001), kao i značajan uticaj na vrijednosti HVU (Sig.=0.000), te značajan uticaj i na veličini IMU (Sig.=0.022). Kod stopala sa deformitetom dominiraju kosa i naglašeno kosa forma prvog MTC zgloba i analiza pokazuje statistički značajnu vezu sa sa IMU (Sig=0.025) i uticaj na vrijednosti ovog ugla (Sig.=0.005) ali i značajnu korelaciju sa HVU (Sig.=0.015) i nešto manji uticaj na vrijednosti ovog ugla (Sig.=0.008). Kod stopala sa deformitetom dominira lateralna inklinacija zglobne površine baze prve MT kosti (76.0%) a kod 10.2% je utvrđena medijalna inklinacija i dokazana je njena statistički značajna povezanost sa IMU (Sig.=0.002), kao i značajan uticaj na vrijednosti IMU (Sig.=0.000) dok na HVU nemaznačajnog uticaja...</dc:description>
  <dc:description xml:lang="eng">Objective: When considering the etiology of hallux valgus deformity, more and more attention is being paid to endogenous etiological factors relating to the morphological specificities of the bone and joint structures of the first ray of the forefoot. This research is aimed to the analysis of the influence of the length ratio of the first and second MT bones as well as the morphology of the first MT bone at the distal level and the possible influence of the morphology of the first MTC joint on the increase of HVA and IMA that define the severity of this deformity.Material and method: 167 patients with hallux valgus deformity and 111 subjects without this deformity were included in the prospective study. After the clinical treatment, standardized radiographic imaging of the feet and the necessary measurements were performed using a licensed digital program. Measurements of the lengths of the first and second MT bones distal to Maestro&apos;s line were made, the radiographic form of the head of the first MT bone and the position of its articular surface were determined by measuring DMAU, the orientation of the base of this bone and the degree of inclination of its articular surface by measuring PMAU. The form of the first MTC joint was determined by measuring the DCAU and measured HVA, IMA and IFA, as well as the absolute length of the first MT bone, proximal and distal phalanx of the big toe. By applying appropriate statistical analyses, the relation of each of the mentioned independent variables with the values of HVA and IMA was examined, and then their influence on the size of these angles.Results: A statistically significant correlation was found between the difference of the lengths of the first and second MT bones with the IMA, as well as a significant influence on the values of this angle at the p=0.01 level, while the influence on the HVA was not confirmed. The round shape of the head of the first MT bone is present in 82.6% of feet with deformity, while in healthy ones there is an equal representation of the round (43.2%) and square shape. (42.3%). A statistically significant relation was shown between the shape of the head of the first MT bone and HVA (Sig.=0.004), but also a significant influence on the values of this angle (Sig.=0.000), as well as a less significant relation with IMA (Sig.=0.012) and the influence to its size (Sig.=0.047). Unlike healthy feet, feet with deformity are dominated by a moderate and strong lateral angulation of the articular surface of the head of the first MT bone, which has a statistically significant correlation with HVA (Sig.=0.001), as well as a significant influence on HVA values (Sig.=0.000), and significant influence on the size of the IMA (Sig.=0.022). In feet with deformity, the shape of the first MTC joint is dominated by oblique and strongly oblique forms, and the analysis shows a statistically significant correlation with the IMA (Sig=0.025) and the influence on the values of this angle(Sig.=0.005), but also a significant correlation with HVA (Sig.=0.015 ) and a slightly smaller influence on the values of this angle (Sig.=0.008)...</dc:description>
