    <ns1:description language="sr">Uvod. Dva najcesca oboljenja žucne kese jesu kalkuloza i neoplazma žučne kese. Procenjuje se da je kalkuloza zučne kese prisutna kod 10-18% adultne populacije u svetu. Kalkuloza žučna kese je najčešći gastroenterološki i hirurški razlog prijema na hospitalno lecenje. Evaluacija težine bolesti sada se pravi uglavnom na bazi protokola (Tokijski protokol 2007, dopunjen 2013) . Na osnovu sprovedenih kliničkih ,laboratorijskih, radiografskih dijagnostičkih procedura, kao i praćenja proinflamatornoh, antiinflamatornih i markera oksidativnog stresa kod klasične (konvencionalne) hirurgije i minimalno invazvine (laparoskopske) hirurgije zučne kese otvaraju se nove mogućnosti u kvalitetu i načinu lečenja oboleleih od ove vrste bolesti.
Oslobađanje citokina
TNF-α se oslobada iz monocita, kao odgovor na prisustvo endotoksina u cirkulaciji. Značajan je medijator akutne faze inflamacije.
IL-1b je proinflamatomi citokin, koji se produkuje u toku zapaljenskih procesa odgovornih za vecinu simptoma kod sepse.
IL-2 predstavlja citokin koji stvaraju antigenom aktivirane T ćelije i koji deluje autokrino stimulišući proliferaciju T ćelija, a takođe potencira rast i otpornost regulatornih T ćelija.
IL-4 predstavlja citokin koji najviše stvara Th 2 ćelijska subpopulacija CD 4 ,,helper“ limfocita čija je osnovna funkcija da pokrene diferencijaciju Th 2 ćelija od CD 4 prekursora, da stimuliše B ćelije u stvaranju IgE antitela i IgG1 kao i da suprimira funkcije makrofaga koje zavise od posredovanja interferona γ

    <ns1:description language="en">Introduction. There are two most common of gall bladder are calculosis and neoplasms. It is estimated that calculosis is present in between 10 I 18% of adult population worldwide. Gallstones are the most common gastroentherological and surgical reason for hospital admission. Evalution of serious consequences is based on 2007 Tokyo Protocols which was reevaluated in 2013. On the basis of clinical, laboratorical, ‘and radiographic diagnostic procedures as well on evaluation of pro and antiinflamatory markers as well as markers of oxidative stress in open(classical) and minimally invasive(laparoscopic) surgery of gallbladder there can be created new possibilities in quality and way of treating patients with gallstone disease. Cytokine secretion TNFα is released by monocytes as a consequence of presence of endhotoxins in bloodstream. It is significant acute phase mediator. IL 1β is a proinflammatory cytokine and is responsible for most sepsis linked symptoms and immunological changes. IL 2 is a cytokine created by antigen stimulated T cells whose autocrine effect causes further proliferation of T cells and also enhances T cell growth and resistance. IL 4 is a cytokine secreted by Th 2 CD4 T cell subpopulation whose basic function is to commence Th 2 cell differentiation from CD 4 precursors, to stimulate B cells to create IgE and IgG1 antibodies and to decrease IFN γ stimulated macrophage functions.
